For 1.5 hours you explore Frank Gehry's building, works from the collection (such as Richard Serra's The Matter of Time) and the current temporary exhibitions, with explanations tailored to your interests. The ticket is included and afterwards you can stay in the museum as long as you like.
There's a second 4-hour option that adds a walk around Bilbao with the same guide.
